3	Copy of the School Report Card of the previous academic year (If applicable) (To be self-attested or attested by the parents)
4	Passport size photograph -5
5	Candidate’s Aadhaar Card (To be self-attested or attested by the parents)
6	Passport Copy (If applicable) (To be self-attested or attested by the parents)
7	Parents’ Aadhaar Card (To be self-attested)
8	Parents photos – 2 each
9	Proof of residence (To be self-attested by the parents)
10	Health Certificate (To be self-attested or attested by the parents)
11	Blood Group Certificate (Information)
12	Conduct Certificate (Grade-I onwards) (To be self-attested or attested by the parents)
13	Original Transfer Certificate from the previous school (If applicable). (Copy to be self-attested or attested by the parents)
14	Attested copy of the caste/community certificate and minority certificate (If applicable).

All photocopies should be submitted along with the originals for verification. Guardians can sign the papers only if both the parents have passed away/bedridden/not in the country.
